For a travel medicine consultation, please fill out the “Request Form for Travel Medicine Consultation and Vaccination Appointment” and upload it using the following link: Dracoon Upload Link
A doctor from the public health department will then contact you by phone to schedule your consultation.
Please make sure that the uploaded documents are labeled with your name so that we can clearly identify your files.

For travel vaccinations, especially the yellow fever vaccine, please also upload the completed questionnaire via the following link:

A doctor from the public health department will then contact you to discuss the matter further and, if necessary, schedule an appointment.

The yellow fever vaccine is in stock; other vaccines will be procured as needed.
We ask that you pay for the vaccinations in cash.
